Maybe some sensation play? Hot or cold. You can use ice cubes or buy glass dildos which can be cooled or heated.
It may sound tame, but sensation play done well can create quite confusing messages to the brain, which can have the recipient wondering if they’re feeling actual pain or what’s going on (in a good way). It can be a lot of fun and perhaps something a bit different.
