When I was a teenager, approx 14yo, I started writing a story about a person having dreams within dreams.
30 years later, in 2010, a movie with the same plot as my story made over $800 million!
I would go back and ask that kid "why didn't you finish it the damn story before someone else did? You could have been a brazillionaire!!"
I will probably think of a better answer later but this was the very first thing that occurred to me.

Greatest regret - One of them "missed opportunities." A sub, go figure. We totally fell for each other but somehow we both missed the signs that we were into D/s! Everything ended shortly after we realized, but for other reasons. I've always thought "what if we knew sooner?"
Greatest Joy - Finding out that I was part of a BDSM community. That really helped me understand my sexual dynamics and relationships. That realization changed my life.

I have been thinking about this for a few hours and I really can't think of anything interesting. Fortunately the reason is because I have fulfilled most of my reasonable wildest fantasies. So this wont be a very interesting answer:
One of my wildest fantasies has always been about being a professional musician/writer. I am both (though not pro) and I do each well, but neither pays the bills. So it continues to be a fantasy.
